Efecto del tratamiento con ranibizumab intravítreo en los diferentes patrones de edema macular diabético obtenidos por tomografía de coherencia óptica de dominio espectral
Resumen
Objetivo: Comparar el efecto terapéutico en diferentes patrones de edema macular
diabético (EMD) determinados por tomografía de coherencia óptica (OCT), después de la
inyección de ranibizumab intravítreo (RIV) por 3 meses consecutivos.
Métodos: Las historias médicas de pacientes que recibieron RIV durante 3 meses
consecutivos, se analizaron retrospectivamente. Se incluyeron 36 ojos de 24 pacientes con
EMD clínicamente significativo y un grosor foveal Central (GFC) mayor de 250 μ medido
con OCT previo al inicio del tratamiento. Estos se clasificaron en cuatro categorías según el
patrón del OCT: Engrosamiento difuso de retina (EDR), edema macular quístico (EMQ) y
desprendimiento seroso subfoveal (DSS) y anormalidades de la interfase vitreoretininana
(AIVR). La agudeza visual con corrección (AVCC) y el GFC antes y después del
tratamiento con RIV se compararon en base a los patrones.
Resultados: Después de las inyecciones de RIV, la variación de AVCC (Log MAR) fue,
-0,05 + 0,17, - 0,24 + 0,25, -0,30 + 0,08, y la disminución del GFC fue, -104 + 121,58 μ,
-173,38 + 230,65 μ, -288,43 + 204,06 μ en los patrones EDR, EMQ y DSS
respectivamente. Todos disminuyeron significativamente su grosor, aunque esta diferencia
no fue significativa entre los grupos (P = 0,242). Sin embargo aquellos con EMQ
mostraron una ganancia significativamente mayor de AV (P = 0,017).
Conclusión: Todos los patrones mostraron una reducción significativa del GFC pero la
ganancia de AVCC fue significativamente mayor en el EMQ que en otros patrones. Los
patrones tomográficos del EMD pronostican la respuesta al tratamiento con RIV. Aims: To compare the therapeutic effect on different patterns of Diabetic Macular Edema
(DME), determined by Optical Coherence Tomography (OCT), after an intravitreal
ranibizumab (IVR) injection for 3 consecutive months.
Methods: The medical records of patients, who received IVR for 3 consecutive months,
were analyzed retrospectively. 36 eyes of 24 patients with clinically significant DME and
greater central foveal thickness (CFT) of 250 μ OCT measured prior to initiation of
treatment were included. These were classified into four categories according to the OCT
pattern: Diffuse retinal thickening (DRT), cystoid macular edema (CME), serous subfoveal
detachment (SSD) and vitreoretinal interface abnormalities (AIVR). The corrected visual
acuity (BCVA) and the CFT before and after the treatment with RIV were compared based
on these patterns.
Results: After the IVR injections, the variation of the logarithm BCVA (log MAR) was -
0.05 + 0.17, -0.24 + 0.25, -0.30 + 0.08, and the reduction in the GFC was -104 + 121,58 μ, -
173,38 + 230,65 μ, -288,43 + 204,06 μ in the DRT, DME and SSD patterns respectively.
The thickness of all four categories decreased significantly, although this difference was
not significant between groups (P = 0.242). However those with CME showed a
significantly greater gain BCVA (P = 0.017).
Conclusions: All the patterns showed a significant reduction of the CGT but BCVA gain
was significantly higher in the CME than in other patterns. The DME topographic patterns
predict response to the treatment with IVR.
