Caracterización de la osteomielitis en pacientes hospitalizados en el departamento de pediatría

El objetivo de esta investigación fue describir las características epidemiológicas, clínicas, tiempo de evolución, patogenia y paraclínica de la osteomielitis en pacientes hospitalizados en el Departamento de Pediatría del Hospital Universitario de Valera “Dr. Pedro Emilio Carrillo”, Estado Trujillo. Investigación descriptiva, diseño de corte transversal retrospectivo y documental; población de 36 pacientes atendidos desde Enero 2003 a diciembre 2015. Encontrando lo siguiente: Grupo etario más afectado niños de 4 años (13,89%), género masculino (58,33%), procedentes del Municipio Sucre (36,11%) factores predisponentes: Traumatismo previo (39,14%) y celulitis (34,78%) huesos más afectados: Fémur (38,89%), y tibia (33,33%); las manifestaciones clínicas más frecuentes: Aumento de volumen (25,22%) y dolor (22,84%),osteomielitis crónica la más frecuente (66,67%); mayormente los casos fueron secundarios a foco contiguo de infección (94,44%).Valor de cuenta blanca: 6,9-10.800 (58,33%) segmentados 51-75%(44,44%), linfocitos 2,1-20%(44,40%); VSG: 41-60mm/h (53,33%) y PCR: 5-30mg/dl (50%). Primeros hemocultivos negativos (63,89%) solo un hemocultivo sucesivo fue positivo para P.aeruginosa (2,77%). Agente causal más frecuente S.aureus (47,22%), osteomielitis crónica severa reagudizada hallazgo más frecuente en las biopsias (70%). En las radiografías de ingreso y sucesivas el hallazgo más frecuente fue alteración del periostio (38,89%) y (41,67%) respectivamente, tumefacción de partes blandas lo más encontrado en ecografías (47,37%), en tomografía axial elevación del periostio, osteomielitis y tumefacción de partes blandas fue lo más hallado (25%), Cintilograma con Tc 99 osteomielitis aguda y actividad osteoblástica fueron los hallazgos más frecuentes (50%). The aim of this research was describe the epidemiological, clinical, duration, pathogenesis and paraclinical of osteomyelitis in patients hospitalized in the Department of Pediatrics, University Hospital of Valera "Dr.Pedro Emilio Carrillo” State Trujillo.
