Comparación de la efectividad relativa analgésica del Ketoprofeno en combinación con Dexametasona versus monoterapia con Zolmitriptán en el tratamiento agudo de la migraña en pacientes que acuden al Instituto Autónomo Hospital Universitario de Los Andes, Mérida 2013
Resumen
La migraña representa un alto porcentaje de consultas en el servicio de urgencias,
constituyendo causa importante de alteración de la calidad de vida y ausentismo laboral.
Actualmente se orienta el manejo de acuerdo a los conocimientos en fisopatología de la migraña,
siendo los más utilizados los triptanos, pero también otros del grupo AINES.
Objetivo: Comparar la efectividad del tratamiento de la crisis migrañosa con ketoprofeno
más dexametasona versus un triptano como el zolmitriptán.
Método: Se incluyeron pacientes que acudieron al Instituto Autónomo Hospital
Universitario de Los Andes que cumplieran ios criterios para crisis de migraña, con edades
comprendidas entre 18 y 65 años. Se dividieron en 2 grupos; el A recibió ketoprofeno 100 mg
más dexametasona 8 mg por vía intravenosa, y el B zolmitriptán 2,5 mg oral. Se evaluó la
respuesta del dolor y la aparición de efectos secundarios.
Resultados: ingresaron 65 pacientes en el estudio, 34 en el grupo A, y 3i en el grupo B,
con una media de edad de 39 años. Se encontró una frecuencia elevada, 41,50%, de trastornos de
ansiedad y/o depresión. El 76% no recibía medicación profiláctica al momento del estudio. No se
encontraron diferencias significativas en cuanto al alivio del dolor en los dos grupos. Se presentó
con mayor frecuencia efectos colaterales en el grupo B con respecto al A.
Conclusiones: El tratamiento con ketoprofeno más dexametasona tiene una efectividad
similar al zolmitriptán con menos efectos colaterales. Migraine constitutes a high percentage of consults in the emergency department, being an
important cause of alteration in quality of life and occupational absenteeism. Currently the
treatment is oriented in agreement of the knowledge of the physiopathology of migraine, and the
triptanes along with NAID are the most used medicines.
Objective: To compare the relative effectiveness of ketoprofen with dexamethasone versus
a triptan like zolmitriptan in the treatment ofthe acute crisis ofmigraine.
Methods: We included patients that carne to the Instituto Autónomo Hospital Universitario
de Los Andes who fulfil the criteria for crisis of migraine, with ages between 18 and 65 years.
Were randomized in two groups: A who received ketoprofen 100 mg and dexamethasone 8 mg
intravenous versus zolmitriptan 2,5 oral. We assessed the pain response and the ocurrence of side
effects.
Results: 65 patients were avaible for the study, 34 in the group A and 31 in the group B,
with an average of 39 years. We found high frequency of anxiety/depression disorders, 41,50%.
76% didn't take prophylactic medication at the moment of the study. The pain was moderate to
severe. No significative ditIerences were found between the two groups concerning the pain
relief. A higher frequence of side effects were found in the group B.
Conclusions: The treatment with ketoprofen and dexamethane have a similar effectiveness
with the zolmitriptan with less side effects.
