Evaluación del proceso de atención al paciente critico y relación con situación de crisis en el sector salud

Introducción: La carencia de medicamentos y desabastecimiento de insumos en el IAHULA se agravó en los últimos años debido a la crisis económica que actualmente vive Venezuela, esta situación limita la calidad de atención a los usuarios. Objetivo: Evaluar el proceso de atención al paciente crítico en las etapas de admisión, hospitalización, alta y seguimiento durante los bienios 2014-2015 y 2016-2017.Metodología: Estudio descriptivo, longitudinal, dividido en dos períodos donde se revisaron las solicitudes de ingreso a UCI y las historias clínicas de los pacientes que se hospitalizaron en terapia intensiva durante los bienios 2014-2015 y 2016-2017.La interrelación entre las variables cualitativas se evaluó mediante chi cuadrado y entre las variables cuantitativas por prueba de U Mann Whitney. Resultados: la edad promedio de la población fue de 35,72 años, del género masculino 52.3%; de ocupación obrero en un 94,07% ; procedente del área Metropolitana un 51,7% y pertenecientes al estrato IV y V de la escala de Graffar un 87,6%,; con nivel de prioridad al ingreso al SMI apropiado en ambos bienios ( 78,2 y 60,7%) Los pacientes del segundo bienio tenían mayor puntaje de APACHE (21,67 % y 33,8%). La demora la ingreso al SMI fue de 51,9% y 69,2%; la demanda insatisfecha para ambos bienios fue de 40,2 y 60,2%. Más del 60% de los casos no tuvieron atención gratuita con recursos del hospital, en el segundo período, 93% de los pacientes recibieron atención médica con recursos adquiridos por su familia, la frecuencia y tasa de neumonía asociada a ventilación mecánica también se incrementó en este período 22,5 y 37,8 x 1000 días de ventilación mecánica respectivamente. La duración de la hospitalización en cuidados intermedios fue mayor en el segundo período ( 23,2 ±15,9 y 48,4 ± 53,8 ); igualmente los días de hospitalización en la UCI también se incrementaron en el segundo período ( 6,4 y 7,3 ) y el retraso al alta también se incrementó en el segundo bienio ( 0,3% y 24,3% ); el retraso al alta tuvo un aumento marcado en el segundo período 0,35 y 24 ,3%, todos con significancia estadística.Conclusión: los resultados indican disminución de los estándares de calidad en el proceso de evaluación de la práctica asistencial. The patients of the second period of two years had APACHE'S major value (21,67 % and 33,8 %). I deposit the delay to the SMI it was 51,9 % and 69,2 % respectively, with statistical meaning. More than 60 % of the cases did not have free attention with resources of the hospital, in the second period, 93 % of the patients received medical attention with resources acquired by his family, the frequency and rate of pneumonia associated with mechanical ventilation also increased in this period 22,5 and 37,8 x 1000 days of mechanical ventilation respectively. The duration of the hospitalization in elegant intervals was major in the second period (23,2 ±15,9 and 48,4 ± 53,8); equally the days of hospitalization in the UCI also increased in the second period (6,4 and 7,3) and the delay to the discharge also increased in the second period of two years (0,3 % and 24,3 %); the delay to the discharge had an increase marked in the second period 0,35 and 24, 3 %, all with statistical meaning. Conclusions: the results indicate decrease of the qualit standards in the process of evaluation of the welfare practice
