Índice de función sexual femenina posterior a Histerectomía

Abstract

La sexualidad es una situación importante cuando se va a dar un cuidado de salud integral a la mujer expuesta a una histerectomía. Objetivo: evaluar el índice de función sexual femenina en mujeres sometidas a Histerectomía. Materiales y métodos: estudio de tipo descriptivo, observacional de corte transversal y comparativo, realizado en la consulta Ginecológica del Hospital Universitario de Los Andes, entre abril y octubre del 2006. Muestra: 138 mujeres divididas en 2 grupos: grupo 1 (Control) 71 pacientes que no tenían antecedente de cirugía ginecológica y Grupo 2: 67 pacientes con antecedente de histerectomía total abdominal. Instrumento: cuestionario pre-elaborado y 19 preguntas, agrupadas en seis dominios: deseo, excitación, lubricación, orgasmo, satisfacción y dolor. Análisis estadístico: ANOVA según homogeneidad de la varianza, t test para muestras independientes y Chi cuadrado. Resultados: edad media: 44,48 ± 6, 78, La miomatosis uterina (82%) fue la principal causa de intervención. La puntuación del índice función sexual femenina en las pacientes no operadas fue de: 21,2 puntos, mejorando en las operadas a 22,3 puntos, logrando su máxima expresión entre los 30 y 34 años (28±7) cayendo después de 55 años (17±12). La educación secundaria tiene la mejor puntuación (25,7 ± 8,7), descendiendo en las universitarias a (21,9±14,4), siendo precaria en las analfabetas (11,2 ±9,1) (P <0.05). Las casadas presentaron la mejor puntuación (23±8,6) siendo las más perjudicadas las viudas y las divorciadas. Conclusión: la intervención quirúrgica no afecta negativamente la sexualidad, pudiendo considerarse la disminución de esta a mayor edad por otros factores independientes de la histerectomía.

Instrument: pre-elaborated questionnaire and 19 questions, grouped in six dominions: desire, excitation, lubrication, orgasm, satisfaction and pain. Statistical analysis: ANOVA according to homogeneity of the variance, t test for independent samples and square Chi. Results: mean age: 44,48 ± 6,78, the uterine miomatosis (82%) was the main cause of intervention. The score of the index feminine sexual function in the operated patients was not of: 21.2 points, improving in the patients to 22.3 points, obtaining its Maxima expression between the 30 and 34 years (28±7) falling after 55 years (17±12). The secondary education has the best score (25,7±8,7), descending in college students a (21,9±14,4), being precarious in the illiterate ones (11.2±9,1) (P < 0,05). The married ones presented/displayed the best score (23±8,6) being harmed the widows and the divorced ones. Conclusions: the operation does not affect the sexuality negatively, being able to consider the diminution of this to greater age by other factors in depth.
