Morfotipos bacterianos y leucocitos con el aislamiento e identificación de los microorganismos en muestra de orina en adultos

Abstract

Las infecciones del tracto urinario constituyen un importante problema de salud que afecta a millones de personas cada año, se hallan entre las enfermedades infecciosas más frecuentes. El examen microscópico de las muestras clínicas es de gran importancia en la orientación del diagnóstico, además de permitir convalidar la calidad, también permite observar la presencia o no de microorganismos en las muestras, por sus características morfológicas o propiedades de tinción particular. Por tal motivo, el objetivo de esta investigación fue analizar la correlación de los morfotipos bacterianos y leucocitos polimorfonucleares en la coloración de Gram, con el aislamiento e identificación bacteriana en urocultivos de adultos en la ciudad de Valera Estado Trujillo, durante el mes de Marzo del 2019. La investigación fue de tipo analítica, con diseño de campo, la n muestral fue de 150 pacientes. Se realizó análisis de asociación de las variables con intervalo de confianza (IC) de 95% e interpretando el nivel de significancia estadística (α) menor a 0,05. Los valores de validez predictiva obtenidos con la coloración de Gram para recuento de leucocitos polimorfonucleares (PMN) fueron S: 56,7%, E: 58,3%, VPP: 61,5%, VPN: 53,4%, para la identificación morfológica bacteriana se obtuvo S: 55,9%, E: 44,6%, VPP: 47,9% y VPN: 52,6%. La sensibilidad y especificidad del método depende del punto de corte del contaje de leucocitos polimorfonucleares (PMN) y la cantidad de bacterias en el frotis, ya que el recuento escaso de bacterias y conteos menores de 4 leucocitos por campo aumentan los posibles falsos negativos.

Urinary tract infections are a major health problem that affects millions of people each year, they are among the most common infectious diseases. The microscopic examination of clinical samples is also of great importance in the orientation of the diagnosis, to allow validating the quality, it also allows to observe the presence or not of microorganisms in the samples, due to their morphological characteristics or particular staining properties. Therefore, the objective of this research was to analyze the correlation of bacterial morphotypes and polymorphonuclear leukocytes in Gram staining, with the isolation and identification of bacteria in adult urine cultures in the city of Valera, Trujillo State, during the month of March 2019. The research was analytical, with a field design, the sample n was 150 patients. An association analysis of the variables with a 95% confidence interval (CI) was performed and the level of statistical significance (α) less than 0.05 was interpreted. The predictive validity values obtained with the Gram stain for the polymorphonuclear leukocyte count (PMN) were S: 56.7%, E: 58.3%, PPV: 61.5%, NPV: 53.4%, for the Bacterial morphological identification was obtained S: 55.9%, E: 44.6%, PPV: 47.9% and NPV: 52.6%. The sensitivity and specificity of the method depended on the cut-off point of the polymorphonuclear leukocytes (PMN) count and the amount of bacteria in the smear, since the low count of bacteria and counts less than 4 leukocytes per field showed possible false negatives.
