Miniplacas vs. cerclaje de alambre en el tratamiento de las fracturas de mandíbula en el I.A.H.U.L.A.-Mérida

Abstract

Se realizó un estudio clínico de tipo prospectivo concurrente comparándose la efectividad del tratamiento con miniplacas de titanio y el cerclaje de alambre maleable de acero en osteosíntesis de las fracturas de mandíbula (valor de significación del estudio p0.05), evaluada según la escala funcional de Olson en 30 pacientes (15 por grupo), con fracturas de mandíbula mayores de 15 años de edad, que acudieron a la emergencia del IAHULA en el lapso de Agosto 2001 a Marzo 2003. Fueron sometidos a tres evaluaciones clínicas: postoperatorio inmediato, a las tres semanas y a los tres meses. En la primera evaluación se observaron resultados excelentes en el 66,7 % de los tratados con miniplacas y 13,3 % en el grupo con cerclaje y ningún resultado malo (p <0.05). En la segunda evaluación encontramos 100% de resultados excelentes en tratados con miniplacas y 40% excelentes en aquellos con cerclaje y un 6,6% de resultados malos en este grupo (p <0,05). En la tercera evaluación el grupo de miniplacas obtuvo 100% de resultados excelentes y 93,3% en el grupo de cerclaje y ningún resultado malo (p>0.05). El 56,7% tenían edades comprendidas entre 15 y 30 años. El 66,7% eran del género masculino. Los accidentes viales y riñas representaron el 90% de la etiología. La región condilar fue la más afectada en un 40% de los casos. La infección de un caso (6,6%) se observó en los tratados con cerclaje, no hubo fatiga del implante ni pseudoartrosis entre los casos estudiados.

It was carried out a clinical study of concurrent prospective type that compared the effectiveness of the treatment with titanium miniplates and the malleable wire of steel in osteosintesis of the jaw fractures (value of significance of the study p0.05), evaluated according to the functional Scale of Olson in 30 patients with jaw fractures bigger than 15 years of age that they went to the emergency of the IAHULA in the lapse of August 2001 to March 2003. They were subjected to three clinical evaluations (postoperative immediate, to the three weeks and the three months); being observed in the excellent first evaluation in 66,7% of the treaties with miniplates and 13,3% in the group with wire and any bad result (p < 0.05). In the second evaluation we find 100% of excellent results in treaties with miniplates and 40 excellent% in those with wire and 6,6% of bad results in this group (p <0,05). In the third evaluation the miniplates group obtained 100% of excellent results and 93,3% in the cerclaje group and any bad result (p>0.05). 56,7% had ages understood between 15 and 30 years. 66,7% was of the masculine gender. The traffic accidents and fights represented 90% of the etiology. The region condilar was the more affected in 40% of the cases. The infection of a case (6,6%) it was observed in the treaties with wire, there was not fatigue of the one it implants neither pseudoartrosis among the studied cases.
