dc.descriptionThe objective of this article is to carry out a review of the updates of the BI-RADS descriptor system, being that of the year 2013 the fifth and last version of the radiological atlas of images and BI-RADS report. This new version incorporated modifications in the management, nomenclature and descriptors of different lesions and, among these, non-palpable breast lesions. The BI-RADS system can be used in mammography, ultrasound and magnetic resonance imaging. Normally, when the radiologist performs several tests, he assesses them jointly, issuing a single final category, based on the highest level finding observed. The American Cancer Society (ACS) recommends annual screening mammography starting at age 40. For women with a positive family history of breast cancer, annual screening begins at age 30. In this context, the establishment of the BI-RADS categories of the American College of Radiology has been an important milestone in the classification of breast findings, by allowing the establishment of guidelines for their diagnosis. Within the BI-RADS classification, category 1 corresponds to the normal breast, without lesions; category 2 to inconsequential benign lesions; category 3 to probably benign lesions, with a positive predictive value (PPV) for cancer of less than 2%; category 4 to suspicious lesions with a PPV variable between 2 and 85%, and finally category 5, highly suspicious of malignancy, with a PPV greater than 85%. Obviously, it does not make sense to biopsy lesions classified as category 1 or 2, however, it is necessary to biopsy category 4 or 5 lesions given their moderate or high PPV for cancer. In this paper we review the role of each biopsy technique, the BIRADS classification, its importance, as well as its role in non-palpable breast lesions.en-US
dc.descriptionEl objetivo del presente artículo es rea-lizar una revisión de las actualizaciones del sistema descriptor BI-RADS siendo la del año 2013 la quinta y última versión del atlas radiológico de imágenes y reporte BI-RADS. Esta nueva versión incorporó modificaciones en el manejo, nomencla-tura y descriptores de distintas lesiones y, entre estas, de las lesiones mamarias no palpables. El sistema BI-RADS se puede utilizar en mamografía, ecografía y reso-nancia magnética. Normalmente, cuando el radiólogo realiza varias pruebas, las valora de forma conjunta emitiendo una categoría final única, con base en el ha-llazgo de mayor nivel observado. La Ame-rican Cancer Society (ACS, Sociedad Estadounidense de Cáncer) recomienda efectuar un screening mamográfico anual a partir de los 40 años. En el caso de mu-jeres que tienen antecedentes familiares certeros de cáncer de mama, se comien-za el screening anual a los 30 años. En este contexto, el establecimiento de las categorías BI-RADS del Colegio Ameri-cano de Radiología ha supuesto un im-portante hito en la clasificación de los ha-llazgos mamarios, al permitir establecer unas pautas para su diagnóstico. Dentro de la clasificación BI-RADS la categoría 1 corresponde a la mama normal, sin le-siones; la categoría 2 a lesiones benignas intrascendentes; la categoría 3 a lesiones probablemente benignas, con valor pre-dictivo positivo (VPP) para cáncer inferior al 2%; la categoría 4 a lesiones sospe-chosas con un VPP variable entre el 2 y el 85%, y por último la categoría 5, alta-mente sospechosa de malignidad, con unVPP superior al 85%. Evidentemente, no tiene sentido realizar una biopsia de las lesiones clasificadas en las categorías 1 ó 2, sin embargo, es preciso realizar biop-sia de las lesiones de las categorías 4 ó 5 dado su moderado o alto VPP para cán-cer. En este trabajo realizamos una revi-sión del papel de cada técnica de biopsia, de la clasificación BIRADS, su importan-cia, así como su papel en las lesiones mamarias no palpables.es-ES
