| dc.description | Objective: Objective: Cervical epidural abscess has an estimated incidence of 0.2-2 cases per 10,000 hospital admissions, mortality is high, it usually presents clinically with pain, fever and neurological deficit, cervical pain being the most common symptom. The most isolated etiological agent is Staphylococcus aureus in 60-70%, commonly with multiple antibiotic sensitivity. Patients with this pathology present with elevated erythrocyte sedimentation rate (ESR) and C-reactive protein (CRP) acute phase reactants, usually accompanied by leukocytosis. The diagnostic method of choice is nuclear magnetic resonance with gadolinium, with a sensitivity of 90%, prevailing over other similar studies. The early approach minimizes the rate of sequelae and mortality, with the start of treatment in the first 24-36 hours being especially important. Conclusion: The diagnosis of this pathology represents a real diagnostic challenge, since it implies an exhaustive evaluation from the clinical and epidemiological point of view, including its low incidence, as well as paraclinical support. The clinical presentation is non-specific, warranting diagnostic suspicion from the initial moment. The duration of the symptoms and the early resolution (within the first 24-36 hours) minimize the possibility of important sequelae such as persistent neurological deficit. | en-US |
| dc.description | Objetivo: El absceso epidural cervical tiene una incidencia estimada de 0.2-2 casos por cada 10,000 ingresos hospitalarios, la mortalidad es elevada, suele presentarse clínicamente con dolor, fiebre y déficit neurológico siendo el dolor cervical el síntoma más común. El agente etiológico mayormente aislado es el Staphylococcus aureus en un 60-70%, comúnmente con sensibilidad antibiótica múltiple. Los pacientes con esta patología cursan con elevación de los reactantes de fase aguda velocidad de sedimentación globular (VSG) y proteína C reactiva (PCR), habitualmente acompañado de leucocitosis. El método diagnóstico de elección es la resonancia magnética nuclear con gadolinium, con una sensibilidad del 90%, predominando sobre otros estudios similares. El abordaje precoz minimiza la tasa de secuelas y mortalidad, teniendo especial importancia el inicio del tratamiento en las primeras 24-36 horas. Conclusión: El diagnóstico de esta patología representa un verdadero reto diagnóstico, ya que implica una evaluación exhaustiva desde el punto de vista clínico y epidemiológico incluyendo su baja incidencia, además de apoyo paraclínico. La presentación clínica es inespecífica ameritando la sospecha diagnóstica desde el momento inicial. El tiempo de duración de los síntomas y la resolución precoz (dentro de las primeras 24-36 horas) minimiza la posibilidad de secuelas importantes como el déficit neurológico persistente. | es-ES |
