Efectividad del tratamiento multidisciplinario y calidad de vida en pacientes con fibromialgia en el servicio de medicina física y rehabilitación IAHULA

Abstract

La fibromialgia se caracteriza por un dolor crónico generalizado con puntos dolorosos y otros síntomas; conlleva al paciente a la discapacidad y al deterioro de su calidad de vida. Evaluará la efectividad del tratamiento multidisciplinario y calidad de vida en pacientes con fibromialgia en el Servicio de Fisiatría IAHULA. Estudio prospectivo longitudinal y experimental con 32 pacientes femeninas referidas de Reumatología, Medicina interna y Psiquiatría que cumplieron con los criterios clínicos Colegio Americano de Reumatología (ACR). Se cuantificó el dolor con la Escala de EVA y la calidad de vida y Escala EQ-5D antes, durante y después del tratamiento multidisciplinario basado en educación, terapia de relajación, ejercicios aeróbicos de bajo impacto tres veces por semana supervisados de manera progresiva en intensidad y duración por 12 semanas. Los criterios de la ACR obtuvieron una media de 12.59 puntos dolorosos, donde predominó la fatiga, rigidez, sueño no reparador, colon irritable. Se obtuvo que la media de edad fue 47.8ª especialmente casadas, docentes con ansiedad y la depresión. La escala de EVA tuvo una progresión media al inicio de 5,6, durante el proceso 3,8 y 2,69 al final del tratamiento rehabilitador. El test de calidad de vida EQ-5D con prueba de Mc Nemar y t de student p 0.05 con media al inicio 53.39 y al final 79.38. El tratamiento de la fibromialgia bajo un enfoque multidisciplinario es útil para reducir la morbilidad y está orientado en disminuir el dolor y así elevar la calidad de vida.

Fibromyalgia is characterized by chronic widespread pain with tender points and other symptoms. It leads the patient to disability and impaired quality of life. To evaluate the effectiveness of multidisciplinary treatment and quality of life in patients with fibromyalgia in IAHULA Physiatry Service. Prospective longitudinal and experimental study with 32 female patients referred to rheumatology, internal medicine and psychiatry who met the clinical criteria American College of Rheumatology (ACR). Pain was quantified with EVA Scale and quality of life with EQ-5D scale before, during and after treatment based on multidisciplinary education, relaxation therapy, and low-impact aerobic exercises three times week supervised strength progressively and for 12 weeks duration. The ACR criteria were average 12.59 painful spots, in which predominated fatigue, stiffness, unrefreshing sleep, irritable colon. It was found that the average age was 47.8 ª especially married teachers and emotional component as anexitys and depression. The EVA scale had an early average of 5.6, during the process 3.8 and 2.69 at the end of the rehabilitation treatment. The quality of life test EQ-5D with McNemar test and student t p 0.05 with average at baseline and at the end 79.38 53.39. The treatments of fibromyalgia in a multidisciplinary approach are helpful in reducing morbidity aimed at reducing the pain and thus improve their quality of life.
