dc.description.abstract | Objetivo: Determinar la utilidad de la evolución cuantitativa del contaje de leucocitos (WBC) y del VAN como predictores de mortalidad en niños con sepsis. Materiales y Métodos: Estudio observacional, analítico, concurrente, realizado sobre una base de datos colectada prospectivamente en un período de 15 meses, en la UCI pediátrica del Instituto Autónomo Hospital Universitario de Los Andes. Se incluyeron 72 pacientes, 55 con infección al ingreso y 17 con infección adquirida en UCI, a quienes se realizó hematología completa al diagnóstico de la infección y a las 48-72 horas de seguimiento. Resultados: Predominaron los niños con leucocitosis, pero el 36% tenía WBC normal al inicio, neutrófilia en el 64%; a las 72 h, eran normales, el 60% de las WBC y el 50% del VAN. La frecuencia de shock séptico fue ligeramente mayor en los pacientes con neutrófilia (p= 0,403). La mortalidad fue del 12,5%, no se relacionó con la WBC y el VAN al ingreso y con el control a las 72 horas. El PRIMS fue el único factor relacionado con la mortalidad. Conclusión: La WBC y el VAN al inicio y a las 72 horas de seguimiento de la sepsis, no se relacionaron con la severidad, la progresión del SRIS y la mortalidad. Un volumen considerable de pacientes con clínica de infección bacteriana, presenta WBC y VAN dentro de límites normales. | en_US |
dc.description.abstract | Objective: To determine the utility of the quantitative evolution of the leukocyte count (WBC) and ANC as predictors of mortality in children with sepsis. Materials and Methods: An observational, analytical, concurrent study was conducted on a database collected prospectively over a period of 15 months in the pediatric ICU of the Autonomous University Hospital of Los Andes. We included 72 patients, 55 with infection at admission and 17 with acquired infection in the ICU, who underwent complete hematology at the diagnosis of the infection and at 72 hours of follow-up. Results: Children with leukocytosis predominated, but 36% had normal WBC at baseline, neutrophilia in 64%; at 72 h, they were normal, 60% of the WBC and 50% of the ANC. The frequency of septic shock was slightly higher in patients with neutrophilia (p = 0.403). Mortality was 12.5%, it was not associated with WBC and ANC at admission and with control at 72 hours. PRIMS was the only factor related to mortality. Conclusion: WBC and ANC at the beginning and 72 hours after sepsis were not related to the severity, progression of SIRS and mortality. A considerable volume of patients with symptoms of bacterial infection, presents WBC and ANC within normal limits. | en_US |
