dc.description.abstract | Se evaluó el impacto de la crisis hospitalaria en el proceso de atención médica de niños críticamente enfermos admitidos en la Unidad de Cuidados Intensivos Pediátrica del Instituto Autónomo Hospital Universitario de Los Andes. Estudio observacional, descriptivo; se realizó entre el 01 de febrero y el 30 de agosto del 2017, con todos los pacientes que ingresaron a la sala. Se determinó la frecuencia y tipo de interrupciones del proceso de atención médica por déficit de recursos asistenciales, de igual manera se determinó la fuente que proveyó el recurso. La información se obtuvo de los registros clínicos y por entrevista directa con el personal de salud tratante y los familiares. Los datos se analizaron con el programa Statistical Package for the Social Sciences (SPSS v.20.0). Los resultados se presentan en cifras absolutas, porcentajes, medias y desviaciones estándar con asociaciones entre variables clínicas y demográficas, con T de Student y estadística no paramétrica; considerando significativo un valor de p < 0,05. Los 70 pacientes que constituyeron la muestra presentaron interrupciones del proceso de atención, en promedio 21,6 interrupciones por día/paciente (7-35). En 38,6% ocurrieron interrupciones de alto riesgo. Las actividades más afectadas fueron la administración de medicación y los estudios de laboratorio. Los pacientes que fallecieron tuvieron mayor número de interrupciones por día (fallecidos: media = 25,05 interrupciones-día-paciente [DT = 6,39] vs vivos: media = 20,47 interrupciones-día-paciente [DT = 4,82], p = 0,02). Las interrupciones de “alto riesgo” se asociaron con mayor mortalidad, (55,6% vs 7% sin interrupciones de “alto riesgo”, p < 0,001, OR = 16,667; [IC95% 4,121-67,412]). | en_US |
dc.description.abstract | The impact of the hospital crisis in the process of medical care of critically ill children admitted to the Pediatric Intensive Care Unit of the Autonomous University Hospital of the Andes was evaluated. Observational, descriptive study; was performed between February 1 and August 30, 2017, with all the patients admitted to the intensive care unit. The frequency and type of interruptions of the health care process were determined by the deficit of care resources, and the source that provided the resource was determined. The information was obtained from the clinical records and by direct interview with the treating health personnel and the relatives. Data were analyzed using the Statistical Package for Social Sciences (SPSS v.20.0). The results are presented in absolute numbers, percentages, means and standard deviations with associations between clinical and demographic variables with test Student's T and non-parametric statistics; considering a significant value of p <0.05. The 70 patients who constituted the sample showed interruptions in the care process, on average 21.6 interruptions per day / patient (7-35). In 38.6%, high-risk interruptions occurred. The most affected activities were medication administration and laboratory studies. Patients who died had a greater number of interruptions per day (deceased: mean = 25.05 patient-day interruptions [DT = 6.39] vs. live: mean = 20.47 patient- day interruptions [DT = 4.82], p = 0.02).
