dc.description.abstract | La parada cardiorrespiratoria (PCR) en la población pediátrica rara vez ocurre sin signos de alarma y de manera abrupta, más bien es el resultado del deterioro progresivo de la función respiratoria y circulatoria, cuya vía final, será la falla cardiorrespiratoria. La PCR en niños se asocia con pobre supervivencia, aun cuando se realicen maniobras de reanimación óptimas. Si ocurre en el medio extra hospitalario, la mortalidad puede alcanzar el 100%; cuando sucede en el hospital, la mortalidad oscila entre el 72 y 97%. Este estudio retrospectivo, de casos y controles, estuvo dirigido a analizar la asociación entre variables fisiológicas 12 horas antes del paro cardiaco intrahospitalario (PCIH) y el retorno a la circulación espontánea (RCE) en los niños ingresados en la UCI del IAHULA, durante el periodo enero 2014 a mayo 2018. Se incluyeron 70 pacientes incluidos con edades entre 1 y 15 meses; la mayoría de los pacientes ingresaron por sepsis, con un puntaje promedio en la escala PRIMS de 18,7. La proporción de pacientes con RCE fue de 22,9 % en tanto que la supervivencia al alta de la UCI fue de 7,5%. El 71,4% de los pacientes tuvieron al menos 01 variable fisiológica anormal 12 horas antes del PCIH en la UCI. Después del análisis multivariable, la hipotensión arterial sistólica (p 0,002) y la hipotermia (p 0,02) se asociaron de manera independiente con fracaso en el RCE. Se concluye que el PCIH en la UCI del IAHULA es una situación clínica de altísima mortalidad, con pródromos sugestivos previos al evento y la que el estado hemodinámico previo, expresado en el valor de presión arterial sistólica y temperatura corporal, tienen impacto directo en el pronóstico de los pacientes. | en_US |
dc.description.abstract | Cardiorespiratory arrest (CRP) in the pediatric population rarely occurs without warning signs and abruptly, rather it is the result of progressive deterioration of respiratory and circulatory function, whose final route will be cardiorespiratory failure. CRP in children is associated with poor survival, even when optimal resuscitation maneuvers are performed. If it occurs in the extra hospital environment, mortality can reach 100%; when it happens in the hospital, the mortality ranges between 72 and 97%. This retrospective study, of cases and controls, was aimed at analyzing the association between physiological variables 12 hours before the in-hospital cardiac arrest (PCIH) and the return to spontaneous circulation (ROSC) in the children admitted to the ICU of the IAHULA, during the period from January 2014 to May 2018. Included were 70 patients with ages between 1 and 15 months; most patients were admitted for sepsis, with an average PRIMS score of 18.7. The proportion of patients with ROSC was 22.9% while survival at discharge from the ICU was 7.5%. 71.4% of patients had at least 1 abnormal physiological variable 12 hours before PCIH in the ICU. After the multivariate analysis, systolic arterial hypotension (p 0.002) and hypothermia (p 0.02) were independently associated with failure in the RCE. It is concluded that the PCIH in the ICU of the IAHULA is a clinical situation of very high mortality, with suggestive prodromes prior to the event and that the previous hemodynamic state, expressed in the value of systolic blood pressure and body temperature, have a direct impact on the prognosis from the patients. | en_US |
