Factores de riesgo asociados al desarrollo de neumonía complicada en niños que ingresan al Instituto Autónomo Hospital Universitario de Los Andes - Mérida, Venezuela

Abstract

INTRODUCCION: La neumonía constituye una de las primeras causas de morbilidad y mortalidad en Venezuela, especialmente en la población menor de cinco años, por lo que su adecuada identificación y manejo es para el Pediatra una necesidad. Es importante enfatizar en la detección y tratamiento temprano, como estrategia para la prevención de complicaciones. OBJETIVO. Identificar los factores de riesgo asociados al desarrollo de neumonía complicada en los niños que ingresan a la emergencia pediátrica del Instituto Autónomo Hospital Universitario de los Andes – Mérida, Venezuela. MATERIALES Y MÉTODOS. Se realizó un estudio longitudinal, observacional y analítico, durante el periodo comprendido entre Octubre/2016 y Junio/2017, con una población total de 60 pacientes, donde se incluyó niños con neumonía adquirida en la comunidad (NAC) n= 27 y NAC complicada n= 33, mediante la ficha de recolección de datos que contiene los factores de riesgo identificados por la OMS sumado a otros factores descritos en la literatura. RESULTADOS Y CONCLUSIONES. Los factores de riesgo relacionados con la presencia de neumonía complicada son: asistencia a guardería con un OR de 1,875 y condición de referido con un OR 1,378. Con una tendencia importante de desarrollar NAC complicada aquellos niños que recibieron medicación con AINES al inicio de la enfermedad.

INTRODUCTION: The pneumonia constitutes one of the first reasons of morbidity and mortality in Venezuela, specially in the five-year-old minor population, for what his suitable identification and managing is for the Pediatrician a need. It is important to emphasize the detection and early treatment, like strategy for the prevention of complications. OBJETIVE: To identify the factors of risk associated with the development of pneumonia complicated in the children who enter to the pediatric emergency of the Autonomous Institute University Hospital of the Andes - Merida, Venezuela.

MATERIALS AND METHODS. A longitudinal, observational and analytical study was realized, during the period Octubre/2016 and Junio/2017, with a total population of 60 patients, where it was included children by pneumonia acquired in the community n= 27 and complicated NAC n= 33, by means of the card of compilation of information that contains the factors of risk identified by the WHO added to other factors described in the literature. RESULTS AND CONCLUSIONS. The factors of risk related to the presence of complicated pneumonia are: assistance to day-care with an OR of 1,875 and condition of recounted with an OR 1,378. With an important trend of develop complicated NAC those children who received medication with AINES to the beginning of the disease.
