Validacion del PIM", PRISM y PRISM modificado en una UCI de escasos recursos

Abstract

La predicción del riesgo de muerte se basa en la medición de la severidad de la enfermedad al ingreso a UCI, mediante cuantificaciones de variables clínicas y de laboratorio utilizadas en modelos predictores de mortalidad. Objetivo: Comparar el poder de discriminación y calibración de tres modelos (PRISM, PIM2 y PRIMS modificado) para predecir la mortalidad en la UCI del Instituto Autónomo Hospital Universitario de los Andes (IAHULA). Metodología: Estudio observacional, analítico, retrospectivo, con los registros electrónicos de los niños ingresados a UCI entre enero 2016 y diciembre 2019. Resultados: Ingresaron 385 pacientes, la mortalidad fue de 23,4%. Las razones estandarizadas de mortalidad calculadas con el PRIMS y PIM2 muestran que ambos índices subestiman la mortalidad. Los tres índices mostraron bajo poder de discriminación, evidenciados por el área bajo la curva ROC (PRISM 0,678, PIM2 0,659 y PRIMSIII modificado 0,687), así como baja sensibilidad para predecir mortalidad, pero especificidad superior al 96% (predicción de supervivencia). La calibración del PRIMS y el PRIMSIII modificado fue superior a la del PIM2 (p=0,052). La correlación entre los tres modelos fue positiva, moderada y estadísticamente significativa. Conclusión: Tanto el PRIMS, PIM2 y PRIMSIII modificado mostraron bajo poder de discriminación entre sobrevivientes y muertes en la UCIP del IAHULA. Incluir en el PRIMSIII modificado variables clínicas que traducen disfunción orgánica (vasoactivos y ventilación mecánica) y comorbilidades como la desnutrición grave puede mejorar su desempeño en UCIP de escasos recursos.

The prediction of the risk of death is based on the measurement of the severity of the disease upon admission to the ICU, by quantifying clinical and laboratory variables used in predictive models of mortality. Objective: To compare the power of discrimination and calibration of three models (PRISM, PIM2 and modified PRIMS) to predict mortality in the ICU of the Institute Autonomy Hospital Universitario de los Andes (IAHULA). The three indices showed low discrimination power, evidenced by the area under the ROC curve (PRISM 0,678, PIM2 0,659 and modified PRIMSIII 0,687), as well as low sensitivity to predict mortality, but specificity higher than 96% (prediction of survival). The calibration of the PRIMS and the modified PRIMSIII was superior to that of the PIM2 (p = 0.052). The correlation between the three models was positive, moderate and statistically significant.
