Estudio comparativo de la agudeza visual pre y postoperatoria y tipo de cierre anatómico en cirugía de agujeros maculares idiopáticos empleando aire como endotaponador durante el período enero-septiembre del 2011 en la AVAO-UOC

Abstract

Objetivo: Evaluar el uso de aire como taponador en VVPP y posicionamiento por 24 horas en cierre de agujeros maculares idiopáticos estadíos 3 y 4 comparando la agudeza visual preoperatoria y postoperatoria, y el cierre postoperatorio utilizando el sistema de tomografia coherente de dominio espectral Cirrus® en AVAO-DOCA durante el período Enero-Septiembre de 2011. Diseño: Cohorte prospectivo longitudinal. Pacientes: 12 ojos fáquicos con agujeros maculares primarios idiopáticos, estadios 3 y4. Intervención: Facoemulsificación con implante de lente intraocular, vitrectomía vía pars plana, peeling de membrana limitante interna asistida con azul trypan, endotaponamiento con aire, posicionamiento por 24 horas y seguimiento postoperatorio durante 30 días. Resultados: El 33,3% correspondió a agujeros maculares estadío 3 y 66,7% al estadío 4, la media de la edad de la muestra fue 64 años. La agudeza visual lejana corregida (AVLcc) y el logaritmo del mínimo ángulo de resolución (logMAR) mejoraron significativamente en el postoperatorio a los 30 días, siendo mas significativos los cambios en el estadío 4 para todos los tipos de cierre (P<O,OS). Conclusiones: El uso del aire como endotaponador en cirugía de agujeros maculares es un método seguro, eficaz y confiable que al utilizarse en asociación a posicionamiento de la cabeza por 24 horas y peeling de membrana limitante interna en cirugía de VVPP, proporciona excelentes resultados postoperatorios.

Objective: To evaluate the use of air as a buffer in PPVV and positioning for 24 hours in idiopathic macular holes stages 3 and 4 by comparing the preoperative and postoperative visual acuity, and type of c10sure postoperative using coherent spectral domain Cirrus ® in AVAO-UOCA during the period from January to September 20ll. Design: Prospective longitudinal cohort. Patients: 12 phakic eyes with idiopathic macular holes primary, stages 3 and 4. Intervention: Phacoemulsification with intraocular lens implantation, pars plana vitrectomy, internal limiting membrane peeling assisted with trypan blue, air tamponade, 24 hours head positioning and monitoring postoperatively for 30 days. Results: 33.3% were stage 3 macular holes and 66.7% at stage 4, the average age of the sample was 64 years. Corrected distance visual acuity (AVLcc) and the logarithm of the minimum angle of resolution (logMAR) improved significantly after surgery at 30 days, with most significant changes in stage 4 for aH types of closure (P <0.05 ). Conclusions: The use of air as a buffer in macular hole surgery is a safe, e±Iective and reliable procedure when used in association with head positioning for 24 hours and internal limiting membrane peeling in PPVV provides excellent postoperative results.
