Alteraciones espirométricas en pacientes diabéticos tipo 1
Resumen
La Diabetes Mellitus (DM) afecta a varios sistemas orgánicos. El pulmón cuenta con una
extensa red vascular siendo propenso a sufrir daños microvasculares; produciendo
alteraciones en la mecánica ventilatoria y en los volúmenes pulmonares. OBJETIVO:
Determinar las alteraciones espirométricas en pacientes con DM tipo1 que asisten a la
consulta especializada del Servicio de Endocrinología del IAHULA. MÉTODOS: Se
realizó una investigación observacional, de tipo analítico, transversal evaluando medidas
antropométricas, valores espirométricos, saturación arterial y control metabólico.
RESULTADOS: Se incluyeron 70 sujetos, 25 pacientes con DM tipol y 45 controles. El
28 % de los pacientes presentaron alteraciones espirométricas versus 2 % de los controles
p=0,002 siendo la CVF el parámetro alterado con mayor frecuencia, con tendencia a
relacionarse a la duración de enfermedad y el mal control metabólico. Se encontró además
que 80% de los pacientes presentaban en la oximetría un valor menor o igual de 97%
versus 24,4% de los controles p=0,000. CONCLUSIÓN: Los resultados de este estudio,
sugieren afectación leve de la función pulmonar, expresada por alteraciones espirométricas
sin expresión clínica en los pacientes con DM tipo l. La alteración de la CVF fue la que
con mayor frecuencia se encontró. El cual se relaciona con el mal control metabólico y con
tendencia a mayor duración de la enfermedad. La saturación arterial de oxígeno en los
pacientes diabéticos presenta valores menores a los controles aunque se encuentran entre
rangos normales, siendo estadísticamente significativo. Este estudio apoya la hipótesis de
que el pulmón es un potencial órgano diana implicado en las complicaciones crónicas de
los pacientes con DM l. The Diabetes Mellitus (DM) affects many body organs. Lungs have a vast vascular system
which makes it prone to suffer from microvascular damage, bringing as a consequence
sorne alterations in the ventilator mechanics and in the lungs volume. AIM: Determine the
spirometric alterations in patients with type 1DM that receive a specialized treatment at the
Endocrinology Service of the IAHULA. METHOnS: An observational, analytic and
transverse research was made in order to evaluate the anthropometric measurement, the
spirometric values, the arterial saturation and the metabolic control. RESULTS: 70 people
were included; 25 ofthem were type 1DM patients and the other 45 were controls. 28% of
patients presented spirometric alterations against 2% of the controls with a p-value of
0.002. The Forced Vital Capacity (FVC) was the most frequently altered parameter with the
tendency of being related to the duration of the disease and to the deficient metabolic
control. In addition, 80% of patients presented a pulse oximetry level less than or equal to
97% against 24.4% of controls with a p-value of 0,000. CONCLUSSION: The results of
this research suggest slight consequences in the pulmonary function, expressed by
spirometric alterations with no clinical expression in the type 1 DM patients. The alteration
of the FVC was the most frequently found, which is re1ated to abad metabolic control and
to the tendency to last longer. The oxygen arterial saturation in diabetic patients presented
lower values than the controls even when they are within the normal ranges, which is
statistically significant. This research supports the hypothesis that the lung is a potential
target organ implicated in the chronic complications oftype 1DM patients.
